Elmwood Care’s In-House Hemodialysis Program is a partnership with Affiliated Dialysis and their highly skilled staff. The program is overseen by Board Certified nephrologists. Our program is offered 6 days a week, with convenience and ease to admit a patient quickly and accurately.

Our care is overseen by a clinical management team that are comprised of physicians, medical directors, registered nurses, social workers, dieticians, nephrology technologists, patient care technologists and administrators. Our focus is to provide superior outcomes for our residents.

Elmwood Care is committed to providing quality of life for our residents in a comfortable and familiar surrounding, while eliminating concerns over time spent in transportation due to inclement weather.

Elmwood Care’s Admissions Director will provide an on-site assessment and gather all of the necessary paperwork to expedite the admission process.  We accept all forms of payment including but not limited to Medicare, Public Aid, Public Aid Pending, Private Insurances, Veterans Insurance and Private Pay.
